The Field Nurse Supervisor assists the Home Care Manager with the clinical components of the home care program as it relates to the onboarding of new aides and supervision of patient-centered care in the home. The duties include the clinical element of office orientation and evaluation of skills in the home for staff providing direct hands-on patient care. The Field Nurse Supervisor ensures the skills of the field staff match the needs of our clients to ensure and promote safe patient care and ongoing support in the home.

Responsibilities

  • MUST BE WILLING TO COVER THE FOLLOWING AREAS: Quincy, Weymouth, Braintree, Wrentham, Foxborough, Norton, Mansfield, Seekonk, Swansea, Berkley, Dighton, Westport, Plymouth, CAPE
  • Coordinate scheduling of onboarding of new aides and orientation weekly with Home Care Manager
  • Together with the Home Care Manager, visit new referrals for home care to ensure Active Home Care’s ability to safely provide care for homebound clients
  • Review clinical components and posttest with new hires at orientation
  • Ensure material is understood and provide remediation and additional training if necessary
  • Ensure new home care aides that are providing hands-on care are supervised post orientation in the home and skills of the aide match the care needs of the client
  • Elevate any concerns in the home to the Home Care Manager
  • Schedule ongoing annual visits and communicate any changes to the care team
  • Submit documentation on a timely basis

Qualifications

Key Qualifications

  • RN degree
  • 3 year experience in a health care setting, home care preferred.
  • Solid understanding of home care and field staff disciplines
  • Staffing Flexibility
  • Some technology skills
  • Demonstrated ability to build strong relationships
  • High impact communication skills; both written and verbal
  • Strategic thinker with strong attention to detail
  • Self-starter; able to prioritize, meet tight deadlines and manage time effectively
  • Demonstrated ability to problem solve and implement solutions collaboratively.
  • Must pass a CORI
  • Must have a valid driver's license and vehicle.
  • Must have liability insurance.

Experience:

  • Healthcare: 3 years (Preferred)
  • Home Care: 3 years (Preferred)
