Active Day is seeking a Registered Nurse for a dual role that blends in-office responsibilities with field-based clinical support. In this position, you’ll assist with onboarding new Personal Care Aides, provide clinical oversight during orientation, and conduct in-home evaluations to ensure staff are delivering safe, person-centered care that meets each client's unique needs. Your work will help match caregiver skills to client requirements, supporting high-quality care in the home.

Unlike hospital or residential settings, this role offers a manageable, balanced workload with a focus on relationship-driven care. You'll get to know your members, provide the kind of personalized attention you went into nursing to offer, and rarely handle acute care situations. With the support of in-center Aides and a dependable care team, you'll deliver "true care" in an environment where your impact is personal and lasting.

Responsibilities

What you'll Do: Besides working with an AMAZING Team, here are some of the things you will do:  

  • Provide nursing care and administer medication to members according to an individual plan of care, physician's orders, and emergent needs, and document clinical and progress notes on the member's medical record.
  • Perform assessments and observe and monitor members' vital signs and responses to treatments and medications, reporting abnormalities to physicians.
  • Coordinate care with other member care services (i.e., Dietary, Pharmacy, Dental, Rehabilitation, Physical/Speech/Occupational Therapy, Audiology, DME).
  • Assist members with daily functional activities, including but not limited to hygiene, meals, ambulation, arrivals and departures, and social activities.
  • Ensure our members are cared for in a loving and joyful environment
  • Visit new referrals for Home Care to ensure Active Home Care’s ability to safely provide in home care
  • Review clinical components and post-test results with new hires at orientation
  • Schedule ongoing quarterly visits and communicate any changes to Care Team

Qualifications

Besides Having Fun and Loving our Members, Qualifications include: 

  • HS Diploma or Equivalent
  • RN License, CPR certification, and valid malpractice insurance policy
  • Min. one year full time equivalent experience in a licensed health care facility. 
  • Prior experience working with the elderly and/or adults with disabilities strongly preferred.
  • This is a physical job, after all we are ACTIVE Day! As a result you will need to be able to:
    • Push, pull, squat, turn, pivot, transfer and  lift at least 50 pounds. 
    • Sit, Stand and/or Walk for extended period of time while providing physical assistance to members
