Job Title: Lead Personal Support Worker – Home-Based Services

Location: Naperville, IL (Client locations vary, primarily in Will and DuPage Counties)

Job Type: Part-Time | Flexible Schedule

  • Compensation: $27/Hour 
  • Training and non-billable time paid at Illinois minimum wage

  • Mileage reimbursed at current IRS rate


Job Overview:
We are currently hiring a Lead Personal Support Worker to provide compassionate, one-on-one care for children and adults with developmental disabilities through the Home-Based Support Services program (DHS). This role requires a high level of responsibility, flexibility, and a concierge approach to care, ensuring each individual receives exceptional support tailored to their needs.

Responsibilities

  • Deliver direct care and support in client homes and community settings

  • Implement and document client goals, objectives, and behavior plans

  • Promote independence through support with daily living activities (ADLs), such as personal care, household tasks, recreational engagement, and community involvement

  • Ensure client safety at all times—clients must never be left unattended

  • Maintain accurate documentation and data collection each shift

  • Attend and complete required training, including state-mandated DSP certification within 90 days

  • Communicate and collaborate with the client’s support team, family, and Clinical Manager

  • Immediately report medical issues, behavioral concerns, incidents, and any allegations of abuse or neglect

  • Assist individuals who may exhibit challenging or maladaptive behaviors

Schedule:

  • Flexible, variable schedule

  • Morning, afternoon, and evening availability required

Work Locations:

  • Naperville-based office

  • Client homes throughout Will and DuPage Counties (some clients may be located outside these areas)

  • Community settings and other locations as assigned

Qualifications

  • 21 years of age or older

  • Valid driver’s license in good standing

  • Access to an insured, registered vehicle for transporting clients

  • Ability to work with both children and adults

  • Strong communication and documentation skills

  • DSP Certification (or ability to complete within 90 days)

  • CPR and First Aid Certification (or ability to obtain upon hire)

  • Experience with individuals with developmental disabilities or mental health diagnoses

  • Experience working with individuals exhibiting maladaptive behaviors

  • Must pass local and/or state background checks
