The Social Services Coordinator will be responsible for all Social Service Assessments and Summaries and provide case management services to address the psychosocial and socio-economic needs of members and their families or caregivers. The Social Services Coordinator communicates regularly with the Center Director and clinical team to ensure members receive the most comprehensive and quality of care possible. Ensures that all care is conducted within HIPAA regulations and per all state-required laws and regulations.
